Discount Plan Options

We offer two plans that save you money and give you peace of mind.

Option 1 – Pay as you go

10% Discount for cash or check payment at time of service. 5% discount for credit card payment at time of service. Teeth Whitening and dental product purchases (toothpaste, mouth rinse, etc.) are excluded from the discount. This a NO FEE plan, it is pay as you go instead of pre-paying for preventive services. Patients can sign up at time of their appointment.

Option 2 – Dental Savings Plan

Our dental savings plan is designed to provide greater access to consistent, quality dental care at an affordable price.

ANNUAL PLAN FEE
1 family member – $425
2 family members – $700
3 family members – $850
4 family members – $1000
Each additional family member – $200 extra

BENEFITS
Covers 100% of Preventive Care including:
– 2 periodic exams and oral cancer screenings
– 2 cleanings or periodontal maintenance visits
– Yearly Bitewing radiographs
– Full Mouth radiographs (every 5 years)
– Fluoride treatments as needed

*All other procedures receive a 20% discount if paid at time of service including: Fillings, Root Canals, Extractions, Crowns and Bridges, Implants, Sealants, Emergency Visits, Cosmetic Dentistry, Dentures and Partials, additional cleanings and radiographs.

No refunds or backdating. Cannot be used concurrently with another dental insurance plan. Discounts
do not cover referrals to specialists. Discounts cannot be used for dental services covered by Workers
Comp Ins.
